Referrals

Before booking an appointment at our Infectious Disease Clinic, a referral from a family physician, specialist, or nurse practitioner is required.

Referral Requirements

To be eligible for consultation, patients must:

• Be 18 years of age or older

• Have a valid OHIP or other health insurance policy.  

Referral Documentation

Referring clinicians must include all relevent clinical information and all relevant documentation. Incomplete referrals may result in processing delays.
